Mentor Logs
The Mentor Log is an activity calendar used to record the activities a mentor and mentee did each month. When you are finished entering your activities, hit "Submit" to e-mail the log to your Program Advocate.
Please submit a log once per month, whether or not you had contact with your mentee. Please include the type of activity and time together. Also note phone calls and length of time. Indicate "No contact" if you didn't have outings or calls in a given month.
These logs help us stay current and enable us to better support matches. Thank you for your efforts, mentors!
