How Do I Enroll My Child?

There are three main steps to enrolling a child into Mentor Duluth's community based mentoring program.

Intake Procedures

1. First a social worker or parent inquires about enrolling youth(s)into the program. 

2. Then the child enrollment coordinator or site advocate determine eligibility on based on criteria including the following: Single parent family or special circumstances, Low income family, Youth must be 5-18 years old and attending school, Family must reside in Duluth, Hermantown or Proctor areas.

3. If eligible, the parent fills out “Parent/Child” application packet and returns to child enrollment coordinator or site advocate.

4. New families are presented at weekly program advocate meetings and distributed to program advocates based on waiting list size and location of family

5. Program Advocate then contacts parent and sets up an intake interview with parent and child(ren). 

 

Intake Interviews

At the intake meeting all questions and concerns from the family are answered and the match process and benefits of the program are discussed. After the intake interview, the child is officially on the waiting list in Mentor Duluth.  They can now take part in all waiting list activities and receive their YMCA membership.

 

Annual Updates

Once a year (from the date of the intake interview), a meeting will be scheduled with the program advocate to update the application, consent form and the child’s picture.  This also helps Mentor Duluth keep updated on the new activities that the child is interested in to best fit them with a mentor.
